For discharge planners, hospice & home health, elder law, and care managers

Refer a family — and actually hear how it turned out.

I'm Amy. Memory care and Alzheimer's placement is all I do, across North Metro Atlanta — East Cobb, Roswell, Marietta, Alpharetta, Johns Creek, Sandy Springs, Dunwoody. When you send a family my way, two things happen that usually don't: I reach out to them the same day, and I come back and tell you how it turned out.

What you can count on

I contact your family the same day.

You're often working against a discharge clock. I move on your timeline, not mine.

You'll hear how it ended.

Most referrals vanish into a black hole — you never find out whether the family got placed, where, or whether they're okay. I close that loop myself. When the family you sent is settled, and with their okay, I'll let you know how it went.

Memory care is the entire job.

I'm not a national call center buying and reselling leads. I'm one person, in your area, who places families into memory care and Alzheimer's communities — and nothing else. I won't embarrass you in front of the family who trusted you.

I only recommend places I've walked.

Every community I'd put your family in front of, I've toured, met the director at, and seen on an ordinary weekday afternoon when nobody's performing for a visitor. If I haven't been inside, I won't send anyone there.

It runs both ways.

When a family I'm helping needs hospice, home health, an elder law attorney, or a care manager, I send them to the people I trust locally. Refer to me and you're not feeding a one-way pipe — you're part of a two-way network.

One ask before you send the form

Please leave protected health information out of the form. No patient names, diagnoses, or medical details. Just tell me who youare, how to reach you, and how soon — I'll call the same day and we'll talk the family through by phone. It's simpler for you, and it keeps everyone protected.

Refer a family

Please don't include patient names, diagnoses, or medical details. Those belong on the same-day phone call, not in this form.

I read every referral myself. Same-day callback, every time.

Prefer to just call? Most people do — (404) 941-4667. That's my number, not a front desk.

For the family you refer: my services are free to them — a community pays my fee only if they choose to move in, and they can stop working with me at any time. I disclose this up front on every intake, in line with Georgia's senior-living referral disclosure requirements. How I work →
